Declaration

• I understand that I have to be 18 years or older, sign the membership agreement, and must provide a valid credit card upon enrolment in the program.
• I agree to have at least one 60 minutes treatment per month for a minimum of 12 consecutive months and I must complete my monthly Queen Massage Health Care Centre visit at the clinic I have enrolled with.
• I agree that in the case I come more than once in month I will pay the corresponding amount upon receipt of my treatment and to pay the extra fee in difference of the amount I was automatically deducted in case I request for additional minutes per session.
• I understand that if I fail to come to get my treatment in a month, Queen Massage Health Care Centre will give me credit for the next month or the following month thereafter.
• I understand that I will be automatically deducted from the credit card I provided once every month for 12 months the total amount plus applicable charges depending on the amount I enrolled in upon signing up.
• I understand that my membership is non-transferable.
• I understand that if I cancel or terminate my contract before the 12 months agreement, I must sign the Termination of Contract Form, acknowledge that I will be charged Regular price and will pay the difference after deducting the membership price for the treatments I had in my previous visit with Queen Massage Health Care Centre.
